Benefits
Octagon Systems Corporation offers a broad range of employee benefits. Benefit offerings are continually reviewed and changes are made based upon employee needs and input.

Health insurance
PPO plan: Prescription drug coverage, $40 co–payment, 80%–50% split, no referrals. Employees are eligible the first of the month after date of hire.

Dental insurance
PPO plan: Preventative Care: 100%, Basic Care: 80%, Major Care 50%. Orthodontia 50% up to $1,500 Lifetime. $50 Calendar year deductible applies.

Vision reimbursement
100% up to the first $100. No deductible, 50%–50% split after that up to the next $400 with a maximum annual benefit of $300. Employees are eligible the first of the month after date of hire.

Life insurance
200% of annual salary to a maximum of $200,000, $5,000 on employee's spouse, $2,500 on dependent children. All coverage is at no cost to the employee.

Short–term disability
Benefit paid at 100% of an employee's salary to cover periods of health disability. Full–time employees are eligible after one year of service. Length of paid leave is dependent on length of service with the company.

Long–term disability
After 90 days of disability, the policy pays approximately 66% of your current income. Coverage is at no cost to the employee.

Flexible spending account
Employees may put pre–tax dollars aside for medical expenses and dependent care expenses.

401(K) retirement
Tax deferred retirement plan - Employees can choose up to eight funds with varying degrees of risk and return. Employees must have 90 days of continuous service with the company in order to participate.

Paid holidays
Ten paid holidays–New Year's Day, Presidents' Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, day after Thanksgiving, Christmas Eve, Christmas, and New Year's Eve.

Vacation benefits
Full–time employees are eligible to accrue paid vacation time immediately upon hire. Years 1 through 3 employees accrue 10 days per anniversary year. Years 4 through 7, employees accrue 15 days per anniversary year. From the 8th anniversary date of service, employees accrue 20 days per anniversary year. Part–time employees are also eligible to accrue vacation time at a pro–rated rate.

Higher education funding program
After 90 continuous days of full–time employment, an employee may be eligible to receive reimbursement for classes taken that are either part of a degree program or that relate to the employee's current or foreseeable job duties.
